| What is your Child Acceptance Criteria? |

| Children will be accepted into the daycare program after an initial interview between the child/ren, parent(s), and myself. This allows child/ren and parents to become familiar with the setting and also the provider. It may also make the transition more comfortable for your children. Please be aware that childcare can not begin until all the state required paperwork and/or documentation is completed and returned to me to be kept on file. This is very important, so please make every effort to have these completed before the first day of care is scheduled. |

| What is your Child to Caregiver Ratio? |

As a licensed care giver, i may provide care for no more than six children. I can care four children under two but only two children under one.i am also licensed for three before and after school children
These children can be in attendance for no more that three consecutive hours before school and no more three consecutive hours after school. |

| What is your policy concerning the Drop off & Pick Up of Children |
| |
| Parents must notify me should anyone besides themselves be picking up the children. This is a much safer method and eliminates any chance of confusion or harm to the children. Only persons listed on the departure slip may leave with your child, so please keep this list as up-to-date at possible. Parents should also provide information that may be important about the child/children….lack of sleep; ect…. at drop off time. Children must be dressed and ready to start the day. I can not dress your child for you in the mornings, children over the age of 1 should not be dropped off in pj’s/one piece sleepers. |

| Yes, I have been trained and am certified in CPR and First Aid. |

| Do you have parties for Birthdays & Special Occasions? |
| |
| At daycare, we will have treats and/or parties on birthdays and special occasions for your children. All the children love this and it makes them feel special. Parents are not expected to bring gifts or treats, please feel free to provide a treat to share, (cupcakes ect..) with the children on your child’s birthday. Please inform me ahead of time. |

| How do you feel about Parent Participation? |

| State Regulations permits on demand access to this daycare during our hours of operation. You are encouraged to stop by anytime your child is here, even if it is just to say hi. Open door policies are only in effect when your child is in daycare. If you need to meet with me after your daycare hours, please call first. |

| What kind of food program do you offer? |
| |
Nutritious meals will be provided for breakfast and lunch for children one year and older. Special diets or food will be provided by parent or legal guardian.
Breakfast is served at 8:20 daily.
Lunch is served at 12:00 noon daily. |

| Do you offer Infant Food? |
| |
| If your child is under one year of age,you need to bring formula, bottles and baby food or other food that your child eats. After his or hers first birthday, I will provide the food unless your child is on a special diet.Then you are expected to continue bringing the food. There should always be a two extra day's supply on hand for emergency use ( as part of our disaster preperation plan). When your physician feels your child is ready for solids foods or weaning, we can plan this together. |

| How Important is the safety of our children at Lucky Ducky? |

| At my daycare, I want to assure you as parents that your child’s safety is taken very seriously. We practice safety here…I have installed outlet covers and all cabinets within a child’s reach that could possibly pose a hazard are safety locked. We have a gate at the bottom of your staircase and one at the exit of our play area inside. Smoke detectors are located on each level of our home, along with fire extinguishers near the furnace and one in the kitchen near the stove. 2 first aide kits are also placed with in the daycare. Monthly fire drills are practiced in the event of a fire. A list of emergency numbers is posted by the telephone. In addition to this, it is important that I have an emergency number for an alternate person in case you can not be reached. Please give prompt notice of change in address or telephone numbers at home or at work. |

| Do you offer Outside Play? |

| The children will be outside as much as possible. We try to get out at least everyday during the warmer weather, and as often as possible in the spring month and right before summer is over is our most nicest weather. Please provide appropriate outside clothing for your child depending on the season. During summer months, when the sun is blazing, I will need for you to provide sunscreen for your children labeled with their names. We like to play with water toys during the hot summer months and swim suite/swim diaper and a towel must be provided for your child to participate. |
